Procare Hospice provides hospice care designed to provide comfort and support to patients and their families. Hospice is a program designed to meet the particular needs of patients and their loved ones when the patient is diagnosed with a disease when all curative measures are exhausted, and comfort (palliative) care is required. Care is delivered by a team of professionals, including the patient’s attending physician, hospice medical director, registered nurse, hospice aide, social worker, chaplain, and volunteers.

Hospice care can dramatically and positively impact your quality of life. Our goal is to empower you and your family, providing knowledge and information to facilitate decision-making. Our clinical staff members are experts on pain control and symptom management. They are also committed to addressing all your needs on medical, physical, emotional, and spiritual levels.

Who is eligible for hospice?

  • Lives within our geographic boundaries
  • Has a terminal illness and seeks medical, spiritual, psychological, and personal support.

Admission Process

Admitting a patient to our hospice program is a simple process for the attending physician, discharge planner, or social worker. They can call us, and one of our registered nurses will visit the patient and family at home, hospital, or nursing home. We will provide the information about our hospice program and will arrange for the start of the services.

We also provide basic medical equipment, supplies, and medications related to the hospice diagnosis.
